You missed the ombuds office at my alma mater, the Kennedy School of Government at Harvard. KSG ombuds are dual-role (usually staff or student) but otherwise typically practice to IOA standards. The KSG Ombuds Program website is at http://www.ksg.harvard.edu/ombuds/.
